Apical lung cancer on shoulder x-ray

Diagnosis almost certain

Presentation

Right shoulder pain

Patient Data

Age: 50 years

Partially delineated 2cm nodule in the 1st/2nd rib space, inferior to the calcified 1st costochondral junction.

No rib erosion.

Degenerative AC joint.

Persistent nodule-like opacity in the right 1st/2nd rib space.

Left basal atelectasis.

Minor right hemidiaphragm elevation.

Case Discussion

CT was undertaken, and a right apical lung malignancy was identified.

The lung apex on the shoulder radiograph is key review area - this is a fellowship classic case.

The examiner is thinking:
Is the potential new consultant colleague competent?
Do they take care of reporting?
Do they have a system to limit missing pathology?
