Presentation

Abdominal pain.

Patient Data

Age: 40 years
Gender: Female

A collection with irregular enhancing walls is seen in the right iliac fossa surrounded by multiple bowel loops. Gas bubbles and an appendicolith are noted within. 

A cecal appendix is identified, reaching a diameter of 15 mm, and accompanied by inflammatory changes in the adjacent adipose tissue.

Remaining small bowel and large bowel loops have a regular parietal enhancement pattern and normal wall thickness. 

There are also changes from previous resection of a pancreatic cyst.

The uterus is of normal morphology according to the patient's age. Two bilateral unilocular adnexal cystic lesions measuring 2.9 cm - 3.3 cm right and left, likely functional. 

Case Discussion

The findings described at the level of the right iliac fossa are compatible with complicated acute appendicitis with appendicular abscess.
