The MRI sequences demonstrate a marked dilatation of the 3rd and lateral ventricles with mild periventricular interstitial oedema, indicating obstructive hydrocephalus. The sagittal FIESTA sequence shows an aqueductal web or diaphragm with normal size and configuration of the 4th ventricle.

Aqueductal stenosis is considered as the most common cause of congenital obstructive hydrocephalus, It can also be seen in adults as an acquired abnormality as in this case.
