Aqueductal stenosis due to developmental venous anomaly

Case contributed by Ammar Haouimi
Diagnosis almost certain

Presentation

Chronic headaches. A VP shunt was inserted 5 years ago and removed 3 years later (surgical report not available).

Patient Data

Age: 30 years
Gender: Female

Marked dilatation of the 3rd and lateral ventricles with a vascular structure obstructing the proximal portion of the aqueduct of Sylvius seen as a developmental venous anomaly on postcontrast T1 sequences as well as on sagittal T1 and T2 as a signal void vascular structure. Normal size and configuration of the 4th ventricle.

Right frontal gliosis probably along the removed VP shunt.

Case Discussion

MRI features of obstructive hydrocephalus due to a developmental venous anomaly obstructing the proximal portion of the cerebral aqueduct (of Sylvius).

Aqueductal stenosis caused by a developmental venous anomaly is very rare, only a few cases were reported in the literature 1,2.
