Presentation
Chronic headache. No history of trauma.
Patient Data
Moderate to marked dilatation of the lateral ventricles and third ventricle. There is no transependymal edema suggesting its chronic course. The fourth ventricle, however, is within normal size.
The proximal segment of the cerebral aqueduct (of Sylvius) is dilated. There is a small membrane below the third ventricle at the distal part of cerebral aqueduct consistent with aqueductal web.

Case Discussion
Aqueductal stenosis due to web/diaphragm is a rare congenital condition in which there is a membrane obstructing the normal flow of the ventricular system between the third and fourth ventricle. Notice the normal fourth ventricle size. The patient usually presents with a headache in a chronic setting.
