Diagnosis certain

Presentation

Vertigo.

Patient Data

Age: 13 years
Gender: Male

There is a 15 x 23 mm well-circumscribed CSF intensity lesion at the right cerebellopontine angle following the CSF signal on all sequences including diffusion-weighted imaging.

The seventh and eighth cranial nerves are displaced superiorly.

No solid elements. No obvious mass effect on the pons.

The ventricles are unremarkable in appearance with no evidence of a hydrocephalus.

The remainder of the intracranial appearances are normal.

Case Discussion

Imaging features are pathognomonic of an arachnoid cyst, in a typical location for an epidermoid cyst. The absence of any significant diffusion restriction confirms the diagnosis.
