CSF intensity space anterior to the cervical cord is associated with distortion of the cord. Prominent CSF flow signal artefact is seen within the abnormality, consistent with a spinal arachnoid cyst. 

Case Discussion

Spinal arachnoid cysts located mostly anterior to the spinal cord are unusual; most spinal arachnoid cysts are in the thoracic region and located posterior to the cord.
