Tracheostomy, thick secretions initially blood-stained, desaturated, new hypoxia despite nasal high flow oxygen. Background of known SCID and CHARGE syndrome.

Tracheostomy in situ. Bronchial wall thickening bilaterally with perihilar changes (right > left) consistent with infection +/- aspiration.
