Presentation

Presents with acute altered mental status.

Patient Data

Age: 85 years
Gender: Female

1st CT emergency department

CT demonstrates bilateral paramedian thalamic and mild rostral midbrain hypoattenuation.

Three days later

ct

A repeat CT 3 days later, demonstrates a more evident bilateral paramedian thalamic and rostral midbrain infarction.

Case Discussion

The imaging differential of bithalamic lesions is broad and includes arterial and venous occlusion, infiltrative neoplasm, and infectious and inflammatory lesions. This case, with acute history, clinical evolution and a new CT scan in a few days, confirms an artery of Percheron infarct
