Ascending colon diverticulitis

Presentation

Right sided abdominal pain

Patient Data

Age: 50 years
Gender: Female

Approximately 5 cm of mid transverse colon shows circumferential mural thickening and mucosal hyperenhancement with inflammation of a couple of diverticuli, localized pericolonic fat stranding. A couple of inflamed diverticuli also seen close to the hepatic flexure. No diverticuli in the remaining large bowel. No suggestion of any underlying neoplasm. A few reactive pericolonic lymph nodes are present.

Moderate fatty infiltration of the liver. 8.8 mm calcified granuloma in segment VII.

Conclusion

  • Features suggestive of active ascending colon diverticulitis.

  • Moderate fatty liver.

Case Discussion

Another case of colonic diverticulitis that appeared as a suspicious mass on colonoscopy. Atypia and malignant changes are more common in diverticulitis and should be a less likely differential. The lack of any asymmetry in the mural thickening tips the diagnosis towards a benign diverticulitis rather than a neoplasm.
