Aseptic loosening of elbow arthroplasty with recurrent trauma

Presentation

The patient experienced a fall down the stairs. She presents with a comminuted fracture. There is a history of a prior elbow arthroplasty.

Patient Data

Age: 80 years
Gender: Female

Total elbow replacement.

Extensive osteolysis surrounding the prosthesis, most pronounced at the humeral component.

There is a sub-acute periprosthetic fracture at the lateral cortex of the distal humerus, this was not apparent in the immediate post-operative imaging below.

Pre-surgical intervention

x-ray

Markedly comminuted fracture of the distal humerus, with dislocation at both the ulnohumeral and radiocapitellar joints.

There is peri-articular edema and soft tissue swelling.

Post-operative imaging of the total elbow replacement confirms the absence of any fractures, dislocations or subluxations.
