Asymmetric fatty bone marrow of the petrous apex

MRI study shows an asymmetrical signal of both petrous apices, the right side has signal void (pneumatized with gas) and the left side elicits high signal on T1, intermediate to high signal on T2 and high signal on FLAIR (fatty marrow signal)

Here is a case of asymmetric fatty bone marrow of the petrous apex, one of the "leave me alone lesions". It is an anatomical variation and should not be confused with other petrous apex lesions.
