Widening of the predental space is seen in the lateral flexion view reaching 6.8 mm. There are multilevel mild anterior spondylolisthesis of C3 through C5 creating a stepladder appearance in the vertebral bodies. Diffuse reduced bone mineralization is noted.
Case Discussion
Atlantoaxial joint is frequently involved in patients with rheumatoid arthritis. Erosions and pannus formation cause disruption of the supporting structures and eventually lead to atlantoaxial subluxation or basilar invagination.
