Presentation
A patient with atrial fibrillation admitted with abdominal pain.
Patient Data
Filling defect in the left atrial appendage.
Large cystic lesion in the spleen.
Hypodense areas in the renal cortices (likely infarcts) as a result of thromboembolism.

Case Discussion
Atrial appendage thrombosis is seen in a variety of clinical settings and can result in severe morbidity or even death from embolic events. Thrombus in the left atrial appendage may cause embolism producing ischemic stroke and infarction of other organs including the spleen, bowel and kidneys.
