Atypical caecal volvulus between liver and right hemidiaphragm

Acute right upper quadrant pain

Patient Data

Age: 75
Gender: Female

Caecum has gone from normal RIF position (previous scan) to enter the interspace between liver and hemidiaphragm (current scan), taking the ileo-colic artrey with it. Distended caecum and small bowel (obstruction) with adjacent free fluid = possibility of venous ischaemia in the volved segment. Note how the liver has become grossly distorted by the markedly distended caecum.
