Presentation
Football player with bilateral hip pain.
Patient Data

The patient was found to have sickle cell disease.
Bilateral hip avascular necrosis more advanced on the left side which displays structural collapse and secondary degenerative arthritis. On the right side there is a crescent sign of femoral head with marrow edema extending to the neck. Bilateral hip joint effusion more on the right side.
Case Discussion
Features are consistent with bilateral avascular necrosis of the femoral heads: grade III on the right side and grade IV on the left.
Sickle cell disease is a known risk factor for interruption of the femoral head blood supply, leading to osteonecrosis.
