Avascular necrosis of femoral head

Presentation

Patient with history of osteoarthritis and chronic hip pain and stiffness worse on the left side.

Patient Data

Age: 57
Gender: Female

There is severe superior narrowing of the left joint space. There are associated subchondral acetabular and femoral head erosions. There is fragmentation, flattening and crescentic lucency of the left femoral head in keeping with avascular necrosis. There are established marginal osteophytes of the acetabular roof.

There is severe concentric narrowing of the right joint space. There are associated subchondral acetabular and femoral head erosions. There are established marginal osteophytes of the acetabular roof. There is subchondral sclerosis. No abnormal soft tissue mass or calcification is identified.

The SI joints appear normal. 

Case Discussion

The appearances are in keeping with avascular necrosis (AVN) of the left femoral head and associated osteoarthritic degenerative changes.
