Presentation

Gradually developed bilateral hip pain with restriction of movements. No history of trauma is provided.

Geographic altered signal intensity areas with irregularities of the articular surfaces are noted involving both femoral heads. The returning signals are hypointense on T1 and T2 weighted images.

The contours of the femoral heads are distorted more on the left side.

Joint spaces are narrowed. Minimal joint effusion is seen bilaterally.

Case Discussion

Findings are more in favour of avascular necrosis of both femoral heads which is most common site in prone circumstances.
