Avascular necrosis of the talus

Presentation

Chronic right ankle pain

Patient Data

Age: 70 years old
Gender: Female
Modality: MRI

A serpiginous line displaying low signal in T1 and high signal in T2 and STIR is seen surrounding the necrotic area of the dome and lateral portion of the talus. Associated marrow edema of the res of talus and moderate joint effusion is noted.

Case Discussion

Diagnosis: Avascular necrosis of talus. T2 weighted images display a typical double line sign which consisting of a hypointense outer line (sclerosis and fibrosis) and a hyperintense inner line (granulation tissue). No fracture could be identified in this case.
