Anterior cruciate ligament avulsion with meniscal tear

Presentation

Twisting and instability of the knee

Patient Data

Age: 20 years
Gender: Male

The posterior horn medial meniscus shows triangular intermediate signal intensity on PD not reaching the articular surfaces suggesting its degeneration.

The posterior horn lateral meniscus shows a complex linear intermediate signal intensity at the PD with both vertical and transverse component, Also a part of this altered signal became hyper-intense at T2 confirming fluid leakage into the meniscal substance. Features denoting flap tear of posterior horn of medial meniscus.

The medial articular surface of the patella shows irregularity with subarticular sclerosis suggesting early osteoarthritis of the patello-femoral articulation.

The tibial plateau shows a hyperintense T2 and STIR line extending at the anterior cruciate ligament attachment site with surrounding bone marrow edema and a separated bony fragment attached to ACL lower end. Features suggestive of recent ACL avulsion from tibial insertion. 

Case Discussion

Surgical repair was performed with confirmation of the meniscal tear.
