Presentation

Twisting injury while playing football.

Patient Data

Age: 15 years
Gender: Male

Same day knee xray

x-ray

Displaced avulsion fracture of tibial apophysis and proximal metaphysis and epiphysis of anterior tibia involving the physis (Type 3 Modified Ogden).

Patella alta.

Suprapatellar joint effusion/haemarthrosis.

Soft tissue oedema.

Avulsion fracture of the tibial tuberosity apophysis through the physis and proximal metaepiphysis (Type 3 Modified Ogden) with marked intervening oedema. Patella alta due to unopposed quadriceps pull. Medial patellar retinaculum tear. Intact patellar tendon with distal insertion onto avulsed tibial tuberosity apophysis still intact. Intact quadriceps tendon. Marked surrounding soft tissue and subcutaneous oedema. Intact cruciate and collateral ligaments. Normal menisci. Traumatic joint effusion.

2 days post ORIF knee xray

x-ray

Post avulsion fracture reduction with four non cannulated lag screws.

Intra-articular drain.

Patella alta.

Subcutaneous and soft tissue oedema.

Congruent distal femur and proximal tibia.

Features of a proximal anterior tibial and apophysis avulsion fracture (Type 3 Modified Ogden) after a twisting injury while playing football.

The patient subsequently had an open reduction and internal fixation with non cannulated lag screws and is recovering well post operatively.
