Avulsion of radial collateral ligament of thumb

Diagnosis almost certain

Presentation

Left thumb injury about 2 -3 weeks back. Complaining of pain and swelling on the radial aspect of the thumb metacarpophalangeal joint.

Patient Data

Age: 25 yrs
Gender: Female

Symptomatic thumb - radial collateral ligament

  • avulsion at the proximal attachment site
  • tiny bone flake at the avulsed end of the ligament
  • dynamic stress scan showed a movement of avulsed bone flake favoring detachment of the ligament.
  • the ligament is deep to the overlying abductor aponeurosis

Symptomatic thumb - ulnar collateral ligament

  • intact and normal echo pattern

Asymptomatic thumb - radial collateral ligament

  • intact and normal echo pattern

Case Discussion

Ultrasound findings favor avulsion of proximal attachment of the radial collateral ligament of the thumb at the metacarpophalangeal joint. The ligament is deep to abductor aponeurosis meaning there is no 'Stener-like' radial collateral ligament lesion. Radial collateral ligament injury is less common than the ulnar collateral ligament of the thumb.
