Presentation

Gymnast female, asymptomatic. Incidental finding (left-sided knee trauma).

Patient Data

Age: 10 years
Gender: Female

Focal defect with surrounding sclerosis in the medial supracondylar femur.

There is a little cortical lesion in the posterior and medial of the supracondylar femoral metaphysis, located at the attachment of the medial head of the left gastrocnemius muscle. The lesion has low signal on T1 weighted-images and shows mild adjacent bone edema on STIR and PDFS sequences. 

Case Discussion

Avulsive cortical irregularity (ACI) is a benign reactive fibro-osseous proliferation due to chronic traction of the adductor magnus aponeurosis or the medial head of the gastrocnemius to their attachment. ACI is a self-limited condition that heals with skeletal maturity.
