B cell lymphoma - perirenal

Presentation

Incidental finding of mild serum creatinine elevation. An abdominal ultrasound was requested as part of the study.

Patient Data

Age: 50 years
Gender: Female
Ultrasound

A slightly hyperechogenic semilunar mass with a hypoechogenic rim is seen adjacent to the upper pole of the right kidney, with no apparent mass effect over the kidney.

The rest of the ultrasound was unremarkable.

CT

An enhancing perirenal mass located adjacent to the upper medial third of the right kidney. The disposition of the mass resembles a subcapsular collection. However, the mass has no mass effect over the kidney.

Case Discussion

With the CT findings, the possibility of a lymphoproliferative disorder was the first impression. Other possible peri-renal etiologies such as Erdheim-Chester and Rosai-Dorfman diseases were also considered.

An ultrasound-guided percutaneous biopsy was performed and the pathology report of a B cell lymphoma was confirmed.
