Bacteroides bacteremia. Acute renal failure.
Patient Data
Intravenous contrast withheld due to acute renal failure.
Large pelvic mass lying between the bladder and rectum. Multiple hepatic masses. Left iliac chain and para-aortic lymphadenopathy. Small volume ascites and omental fat stranding.
Left hydronephrosis. Left adrenal mass.
Case Discussion
The imaging findings are suspicious for metastatic colorectal cancer, especially in the setting of a Bacteroides bacteremia. Bacteroides bacteremia has a high mortality rate (~25%) with malignancy, in particular, colorectal cancer, being a major association and is often the primary infection site. The patient died before a histological diagnosis was made.
