Balo concentric sclerosis

Case contributed by Dr Narek Matinyan

Presentation

Headache, aphasia, seizures, cognitive and behavioural dysfunction.

Patient Data

Age: 40 years
Gender: Male
Modality: MRI

Some areas of pathological MR signal are seen, the biggest of which is localised in the right parietal lobe. There are stripes of high or low signal depending on the sequence, most likely due to areas of demyelination and myelination (“onion bulb” appearance). On the images of T1 C+ there are no signs of contrast enhancement.

Case Discussion

The MRI of the brain showed areas of pathological MR signal, the biggest of which is localised in the right parietal lobe. There are stripes of high or low signal depending on the sequence, most likely due to areas of demyelination and myelination (“onion bulb” appearance). On the images of T1 C+ there are no signs of contrast enhancement.  This case may represent Balo concentric sclerosis. Confirmatory follow-up is pending.
