Bankart lesion - bony and fibrous

Presentation

Pain and functional limitation in previous anterior shoulder dislocation.

Patient Data

Age: 40 years
Gender: Male

Arthrogram-MRI left shoulder

Sagittal MR-arthrogram demonstrates glenoid fracture and bone detachment with complete tearing of the anterior scapular periosteum located in the 3-6 o'clock position ("bony" Bankart).

This injury extends into the glenoid labrum, superiorly up to 12 o'clock position, with discontinuity of the periosteum ("fibrous" Bankart).

There is also Hill Sachs fracture of the humeral head; also noted is an osteophyte of the inferior margin of the humeral head.

Rotator cuff tendons and ligaments are normal.

1) detachment glenoid bone at 6 o'clock position

2) defect of the glenoid cartilage 6 o'clock position at the level of the humeral osteophyte

3) osteophyte of the lower margin of the humeral head

4) detachment of the glenoid lip from 6 o'clock position to 12 o'clock position

 

Case Discussion

The Bankart lesion may involve the glenoid bone or the glenoid labrum, in both cases the periosteum may be interrupted. Bankart lesions are typically located in the 3-6 o'clock position, where the humeral head dislocates in most cases.
