Base of skull fracture with bilateral Le Fort fractures

Presentation

Fell backwards whilst cleaning windows from 6 metres onto concrete landing on his face.

Patient Data

Age: 22
Gender: Male

Heavily comminuted facial bone and base of skull fractures with bilateral Le Fort fractures. The right side demostrates a Le Fort II while the left is a Le Fort III fracture. The base of skull fracture extends through the body of the sphenoid.
 

Case Discussion

The trauma resulted in frontal extradural haemorrhage with bilateral frontal lobe parenchymal contusions and small to moderate volume subarachnoid haemorrhage extending into the basal cisterns. Minimally displaced C4, C5 and C6 spinous process fractures and superior endplate fractures of T5, T6 and T7.
