Platybasia and basilar invagination

Presentation

Abnormal gait.

Patient Data

Age: 10 years
Gender: Male

Tip of the odontoid is above the level of the foramen magnum (basilar invagination) resulting in significant narrowing of the canal and compression over the cord at this level.

Abnormal flattening of the skull bases is seen (platybasia).

No significant obstruction (no hydrocephalus or syrinx formation) noted.

No tonsillar herniation to suggest Chiari malformation (group I).

Case Discussion

Basilar invagination is a craniocervical junction abnormality where the tip of the odontoid process projects above the foramen magnum.

Basilar invagination is often associated with platybasia as in this case.
