Chronic low back pain.

The MRI images demonstrate a lumbosacral transitional vertebra (LSTV) seen as a right mega transverse process of L5 with pseudo-articulation with the sacrum.

Case Discussion

The presence of a lumbosacral transitional vertebra (LSTV) in a patient with low back pain is suggestive of Bertolotti syndrome, however, this syndrome remains controversial.

According to the Castellvi classification of lumbosacral transitional vertebrae, this case is classified type IIa.
