Long standing lower back pain.

Age: 25 years
Gender: Female

Lumbosacral transitional vertebra with lumbarization of S1, characterized by a broadened right transverse process and pseudoarticulation with the sacral ala.

Case Discussion

Lumbosacral transitional vertebrae are relatively common, occurring in around 25% of the general population. Knowledge of these variants is important to avoid performing procedures or operations at the wrong level.

If there is an association with low back pain, the controversial Bertolotti syndrome may be considered. The Castellvi classification is used for lumbosacral transitional vertebrae, with types II and IV potentially associated with a higher prevalence of back pain.
