Biceps long head tendon tear

Presentation

One day before the presentation, the patient lifted a heavy weight resulting in a left anterior shoulder snap. He developed anterior mid arm bulge. Clinically it was a 'Popeye arm'.

Patient Data

Age: 40 years
Gender: Male
ultrasound

There is discontinuation of the biceps long head tendon in cranial part of the bicipital groove. Rest of the tendon is edematous and hypoechoic. There is sagging of the biceps long head myotendinous junction below the level of pectoralis major tendon insertion. Rest of the rotator cuff is intact.

Case Discussion

A male patient presented with 'Popeye arm' after lifting a heavy object. Ultrasound shows a complete tear of biceps long head tendon. Ultrasound shows a primary sign of tendon tear that is tendon discontinuity. Sagging of the biceps long head myotendinous junction below the pectoralis major tendon insertion level is a secondary sign.

Subpectoral biceps tenodesis was done.
