Bifid biceps tendon

Case contributed by Wadah Ahmed
Diagnosis possible

Presentation

Slightly limited range of motion, mainly abduction, of the right shoulder. No known injuries.

Patient Data

Age: 55
Gender: Female
ultrasound

Two tendons were seen throughout the bicipital groove of the right arm. Mild amount free fluid (FF) also noted towards the distal part of the biceps tendon sheath.

Case Discussion

The sonographic appearance of two separate biceps tendons throughout the bicipital groove, in this asymptomatic patient, is suggestive of a bifid biceps tendon, a rare anatomical variant. In an appropriate clinical setting, a split tear of the biceps tendon might have a very similar appearance though.
