Presentation
Presented with severe abdominal pain one day before admission.
Patient Data
Pancreatic swelling with an unenhanced area in the head of the pancreas in contrast phases. Peripancreatic fat stranding, peripancreatic fluid collections which extending into the periduodenal space and perirenal space.
Bilateral adrenal gland hyperenhancement in the arterial phase.
Hepatic steatosis with diffuse hypoattenuation parenchyma.
Small volume of free fluid.
Case Discussion
This necrotizing pancreatitis case presented bilateral adrenal gland hyperenhancement which is an unusual feature. The CT finding is a part of the hypoperfusion complex, an indicator of hypovolemic shock.
This CT finding also can be seen in patients with hemorrhagic shock, sepsis, and trauma.
