Bilateral benign adrenal adenomas

Presentation

Work up for abdominal pain.

Patient Data

Age: 55 years
Gender: Female

A 34×25 mm mass is noted at right adrenal gland. Its mean attenuation values on non-contrast images measured 2HU. After contrast media administration the mass shows rapid washout.

Additionally, a 16×14 mm nodule is also observed at left adrenal gland. Its mean attenuation values on non-contrast images measured 6HU and shows rapid washout on Post contrast CT images. 

Case Discussion

Bilateral adrenal masses. Mean attenuation values and enhancement pattern of the masses are compatible with benign adenoma.

CT is often the modality which identifies an adrenal mass. Fortunately, density evaluation of an adrenal lesion is highly sensitive and specific as 70% of adrenal adenomas contain significant intracellular fat. Lipid-poor adenomas are more difficult to diagnose as the CT density increases and approach that of soft tissue.
