There is increased angle between femoral neck and shaft bilaterally indicating coxa valga.

Case Discussion

Coxa valga is a condition in which both hips have an increased angle between the neck of the femur and the shaft of the femur. This results in the femoral heads being displaced medially and superiorly, and the knee joints being angled outward.

On imaging, the following features can be seen:

  1. Increased femoral-neck-shaft angle: The angle between the femoral neck and shaft on an anteroposterior radiograph of both hips is usually greater than 135 degrees in bilateral coxa valga.

  2. Medial displacement of the femoral head: The femoral head appears to be displaced medially on radiographic images of both hips.

  3. Wider femoral neck: The femoral neck appears to be wider than normal on radiographic images of both hips.

  4. Widening of the hip joint space: The joint space between the femoral head and the acetabulum may appear to be wider than normal on radiographic images of both hips.

  5. Increased acetabular anteversion: The acetabulum may appear to be more anteverted on radiographic images of both hips.
