Bilateral femoral neck stress fractures

Presentation

Athlete with 1 year history of bilateral groin pain.

Patient Data

Age: 17 years
Gender: Female

Bilateral transcervical fracture line of the femoral neck seen as a linear low signal extending from the medial to the superior femoral neck within an area of bone marrow edema of high signal on T2-STIR sequences. Note mild bilateral joint effusion with edematous infiltration of the adjacent soft tissues.

Case Discussion

Femoral neck stress fracture is a rare condition, especially in its bilateral form, and is usually observed in high-level athletes. MRI is the best modality to detect the fracture line and its extension, as well as the edematous infiltration of the adjacent soft tissues and to exclude a cartilaginous or ligamentous injury.
