Bilateral hip avascular necrosis

Presentation

Left nerve root pain.

Patient Data

Age: 55 years
Gender: Male

Lumbar epidural lipomatosis compressing the thecal sac.

On the localizer images, bilateral hip avascular necrosis was suspected especially on the left side.

Coronal STIR images were subsequently performed to confirm the diagnosis.

Bilateral hip avascular necrosis is seen with extensive bone marrow edema of the left hip head and neck and also in the superior aspects of the glenoid cavity.

Case Discussion

This case highlights the importance of observing the scout images. Bilateral hip avascular necrosis was suspected on the scout images, especially on the left; subsequent additional coronal STIR was performed and confirmed the findings. MRI of the hips was recommended for a complete study of the hips.
