Presentation

Follow up chest x-ray. No current symptoms.

Patient Data

Age: 40 years
Gender: Male
x-ray

Bilateral hilar clips and wires from prior clamshell sternotomy noted. 

Minor lateral elevation of the lateral left hemidiaphragm with adjacent architectural distortion of the lung parenchyma in keeping with post-inflammatory scarring. Minor bilateral apical pleural thickening. 

Case Discussion

This patient presented for a routine follow-up radiograph following prior bilateral lung transplantation. Bilateral hilar clips and the configuration of the sternotomy wires (clamshell rather than midline sternotomy) can alert the reporting clinician to the history of prior lung transplantation. 

Minor scarring/post-inflammatory changes at the left lung base are thought to be related to the procedure and recovery period.
