Mature ovarian teratoma - bilateral

- Both ovaries are enlarged by heterogeneous masses with prominent fatty component, within which multiple hyperdense foci are seen.
- There is a walled-off blood density material at the right adnexa.
- The ovaries contain multiple follicles arranged at their periphery.
- No free pelvic fluid.
- Fat planes are not obliterated.
Case Discussion
The identification of macroscopic fat and macro-calcifications within an adnexal lesion is highly suggestive of ovarian teratoma. It is bilateral in about 15% of the patients.
