Case contributed by Abdallah Al Khateeb
Diagnosis certain

Presentation

Severe lower abdominal pain and tenderness. No leukocytosis.

Patient Data

Age: 30 years
Gender: Female
  • Both ovaries are enlarged by heterogeneous masses with prominent fatty component, within which multiple hyperdense foci are seen.
  • There is a walled-off blood density material at the right adnexa.
  • The ovaries contain multiple follicles arranged at their periphery.
  • No free pelvic fluid.
  • Fat planes are not obliterated.

Case Discussion

The identification of macroscopic fat and macro-calcifications within an adnexal lesion is highly suggestive of ovarian teratoma. It is bilateral in about 15% of the patients.
