Presentation

The patient presented to the emergency department with multiple episodes of epilepsy.

Patient Data

Age: 20 years
Gender: Male

Thick and irregular cerebral cortex mainly in the perisylvian region on both sides more on the right with abnormal sulcation. The cortical surface has an irregular flattened appearance.

Bilateral mainly perisylvian cortical thickening with abnormal sulcation more on the right side. The cortical surface has an irregular flattened appearance.

Prominent Sylvian fissure on both sides.

Case Discussion

Polymicrogyria (PMG) is one of many malformations of cortical development, and along with grey matter heterotopias, falls under a group of conditions characterized by abnormalities both in the migration of neurons to the cortex and abnormal cortical organization. The clinical presentation varies depending on the degree of involvement, bilaterality, and associated syndromes. While some patients may have normal neurology, others can have neurodisability or epilepsy which is common in polymicrogyria.

The radiological features of our case are typical of polymicrogyria (PMG) but unfortunately further clinical or imaging follow-up is not available.
