Bilateral synovial facet cysts

Patient Data

Age: 75 years
Gender: Male

Synovial facet cysts are seen at the anteromedial aspects of the right and left L3-4 facet joints, respectively. These cysts are causing severe central canal stenosis, bilateral lateral recess stenosis, and mild compression of bilateral neural foramina.

Grade I anterolisthesis of L4 over the L5 vertebral body is seen. A concentric bulge of the L4-5 disc is seen with ligamentum flavum thickening and gross facetal arthropathy, mildly compressing central canal, bilateral lateral recesses, and neural foramina.

A concentric bulge, and left foraminal protrusion of L5-S1 discs are seen, indenting the thecal sac, moderately compressing the left neural foramina, and mildly compressing the right neural foramina.

Case Discussion

Synovial facet cysts are often associated with facet joint arthropathy and add to the central canal and lateral recess narrowing.
