Almost symmetrical significant bone marrow oedema in the metaphyseal area of both tibiae, defined by serpiginous line, suggestive of metaphyseal stress fracture of the proximal tibia bilaterally. There is surrounding bone marrow oedema in the metaphysis and epiphyseal region of the proximal tibia reaching the overlying articular cartilage with preserved tibia plateau contour is suggestive of the acute stage of the disease.

Case Discussion

In our current case, a young lady with no history of significant trauma developed bilateral metaphyseal fractures of the tibia plateau. Before the examination, the patient reported no history of remarkable disease. This is unusual and could be only explained by stress fractures, which can occur in this location 1,2.

Characteristic findings on MRI include a hypointense line in the bone on T1 weighted imaging, accompanied by extensive bone marrow oedema-like signal intensity.
