Classic trifurcation pattern is interpreted when we have a common confluence of the right posterior, right anterior, and left hepatic ducts. In our case it is a form of complicated trifurcation in which the segment 7 posterior right hepatic duct arises directly from the confluence whereas the segment 6 posterior right hepatic duct arises as a branch from the right anterior hepatic duct. The gallbladder is not visualized as the patient is status post cholecystectomy. Mild right hydronephrosis is seen as well.
