Known for systemic lupus erythematosus treated with corticosteroids.
Patient Data
Serpiginous sclerotic medullary bone lesions involving the distal femur, proximal tibia and patella with central lucencies.
Extensive serpiginous intramedullary lesions involving the distal femur, proximal tibia and patella. They appear isointense to hypointense to the bone marrow on T1 centrally and hypointense peripherally. On PD fat sat sequences, there is a hyperintense inner ring (granulation tissue) with a hypointense outer ring of sclerosis "double-line sign" with peripheral rim enhancement on postcontrast sequences.
Case Discussion
MRI features most consistent with extensive bone infarcts involving the femur, tibia and patella in a patient treated with long term steroid therapy for lupus.
