Boxers 5th metacarpal fracture with previous ORIF for the same fracture

Presentation

Punch injury and painful hand. History of violent behavior.

Patient Data

Age: 25 years
Gender: Male

Plate and screw fixation of the fifth metacarpal shaft and head. Undisplaced transverse fracture of the metacarpal shaft inferior to the fixation plate. No fracture of the metalwork.

Additional oblique fracture of the fourth metacarpal base, with approximately 15 degrees of volar angulation and possible intra-articular extension.

Normal carpometacarpal and metacarpophalangeal joint alignment.

Case Discussion

This patient had a long history of punching people and inanimate objects, with a surgical history of previous 5th metacarpal internal fixation.
