Brachial plexus injury with axonotmesis

Presentation

Road traffic accident.

Patient Data

Age: 20 years
Gender: Male

Hyperintense signal in the trunks, divisions and cords of right brachial plexus with discontinuity of fascicles of right C6 trunk consistent with neuropraxia and axonotmesis (gradeIV: neuroma in continuity).

Small cystic lesion at right C5-6 level with discontinuity of the nerve root consistent with a pseudomeningocele secondary to nerve root avulsion.

Oedema and hyperintensity in right supraspinatus and infraspinatus muscles in keeping with subacute denervation oedema.

Case Discussion

MRI of brachial plexus is of utmost importance in cases of suspected plexus injuries. The patient was involved in a road traffic accident, followed by right arm weakness. MRI reveals findings consistent with neuropraxia, axonotmesis and pseudomeningocele which explain patients symptoms.
