Sharp epigastric pain radiating to the right neck lasting 30 minutes. Radial-radial delay. Mildly elevated D-Dimer.

CT thoracic aorta angiogram reveals a pseudoaneurysm of the brachiocephalic trunk. Enhancing lumen measures 12 mm diameter in its long axis. A narrow neck and surrounding non-enhancing thrombus measuring 24mm x 23 mm. The rest of the neck vessels are otherwise patent.
Case Discussion
This patient had a previous history of type A aortic dissection and repair in 2010.
