Brainstem cavernoma

Diagnosis almost certain

Presentation

Left lateral gaze diplopia for 4 days.

Patient Data

Age: 30
Gender: Male
mri

Evidence of approximately 14 X 11 mm well demarcated rounded lesion at ponto-medullary junction, on posterior medial aspect. The lesion show heterogeneously hyper intense signal on T1WI and "popcorn" like signal on T2WI. The SWI and GRE (T2*) show hemosiderin related susceptibility artefacts in this lesion with a well defined hypo intense rim note on GRE images. No surrounding edema noted. No other lesion noted. No obvious evidence of dural venous anomaly.

The corresponding CT scan image show subtle hyperdensity in the lesion.

The lesion morphology and signal characteristics are consistent possibility of (solitary brain stem) Cavernoma (cavernous angioma)
