Headache, dizziness, and vertigo.
Patient Data
There is a 20*25*30 mm abnormal signal ( high on T2/FLAIR ) mildly expansile solid lesion involving the brain stem with extension to the right cerebral peduncle that shows faint enhancement after contrast injection.
The mass causes mild effacement on the aqueduct canal causing triventricular (lateral and third ventricles) hydrocephaly.
High signal foci in T2/ FLAIR sequences at subcortical and periventricular white matter of both cerebral hemispheres depicts microvascular ischemic events.
Case Discussion
The mass was biopsied and histopathologic examinations revealed:
Microscopic: Sections show blood clots and embedded neoplastic tissue fragments with fibrillary background and low cellularity having uneven distribution.
Diagnosis: Astrocytoma(WHO grade I-II)
